VERIFICATION OF BOARD CERTIFICATION
Managed care organizations, hospitals and health care agencies: To verify a physician's Board Certification, please submit a request in writing along with a signed release form from the physician to the ABPS Executive Offices via fax or mail to the contact below.
Individuals not representing a health organization and requesting board certification should submit a request via fax, mail, or telephone to the contact below.
